it happens, but you have to believe enough and not ask too many questions. also it pays to be a short term pessimist (so you keep worrying and keep working towards the ambition dream goal whatever) and a long term optimist (so you know that what you're working towards will indeed be achieved.) i speak from experience.

Depends. Personally, I think that increasingly, it has become important to incorporate some degree of marketing. Just a slight dollop of style to a solid body of work that should be all substance.
It happens. Maybe late but the fruits will surely turn up.
